You can print and fill out our New Patient Form before your first appointment. Or, if you'd prefer, please arrive 10 minutes early to fill out the form in our office.

Appointments

Your scheduled appointment time has been reserved specifically for you. We request 48 hours of business notice if you need to cancel your appointment to avoid being charged an additional fee.

Payment Options

At your request, your treatment will include a breakdown of all applicable fees, and we will inform you of all costs before treatment is administered. Charges are payable at the time of treatment by Debit Card, Visa, Mastercard, Cheque, or Cash. 

Please ask a member of our staff and we will review your payment options to accommodate your financial needs.

Insurance Benefit Assignment

Dental insurance claims are handled on a “pay patient” basis, also called non-assignment.

Patients are responsible for the payment of dental treatment in full at the time of service and will receive dental insurance reimbursements directly from their insurance carriers for the portion covered by their policy.

We will provide you with non-assignment claims for you to submit and are happy to assist you in securing your reimbursement.
